Insurance
If your plan is a PPO, we likely accept it.
We work with most major PPO dental plans. The most common ones we see include:

Your first visit
Plan for about an hour. Your first appointment usually includes a full exam, X-rays if needed, and a cleaning.
Dr. Mallari will review your dental history, look at anything you’re concerned about, and walk through any recommendations — with the cost, timeline, and options laid out clearly. No high-pressure sales pitches.
Bring your insurance card and a photo ID. If you have records or X-rays from a previous dentist, those are helpful but not required.
Parking & access
We’re located in the plaza at 2382 Crenshaw Blvd, Suite A, in Torrance. Parking is free and plentiful in the plaza lot.

Payment & forms
We accept cash, check, and all major credit cards. A 3% processing fee applies to credit card transactions.
New-patient intake forms are completed in the office on the day of your first visit. We’ll have them ready when you arrive.
